The CRA forms our clients ask for most often. Several of these were renumbered or retired since our original list was published, so each entry points at the form CRA accepts today.
- Authorize a Representative for Offline Access (AUT-01)Replaces the former T1013. Authorizes us to deal with CRA on your behalf.canada.ca
- Business Consent (formerly RC59)CRA retired the RC59. Business authorizations now go through AUT-01 or Represent a Client.canada.ca
- Declaration of Conditions of Employment (T2200)Required from your employer before employment expenses can be claimed.canada.ca
- Disability Tax Credit Certificate (T2201)canada.ca
- Moving Expenses Deduction (T1-M)canada.ca

- Direct Deposit with the CRA (formerly RC366)The RC366 paper form was retired. Direct deposit is now set up online or through your bank.canada.ca
